Health Requirements for School Entry

Per Board of Education Policy 7:100, all students must meet the health requirements as outlined by the Illinois Department of Public Health. Unless an exemption or extension applies, failure to comply with the requirement before September 1 will result in the student's exclusion from school until the required health forms are presented to the District. New students who register after September 1 of the current school year shall have 30 days following registration to comply with the health examination and immunization regulations.

Physical Examination

Immunizations

All students entering grades 6-12 will be required to show proof of recent Meningococcal conjugate vaccination (MCV). All grade 6-11 students must have 1 dose at school entry. Grade 12 must have 2 doses at school entry; 2nd dose must be administered on or after the child's 16th birthday.
Children entering school at any grade level (Kindergarten - 12) shall show proof of having received 2 doses of live virus Measles, Mumps, and Rubella vaccines, the first dose on or after the first birthday and the second dose no less than four weeks (28 days) after the first dose, or other proof of immunity.
Children entering school at any grade level (Pre-K - 12) shall show proof of having received three doses of Hepatitis B Vaccine.
Children entering grades Kindergarten, 1, and 2 must have four or more doses of Polio and DTP/ Dtap vaccine, with the fourth dose given on or after the 4th birthday. Children entering grades 3-12 must have three or more doses of Polio and DTP/Dtap vaccine, with the third dose given on or after the 4th birthday.
Children entering school at any grade level (Kindergarten - 12) must show proof of having received 2 doses of Varicella (chickenpox vaccine) or statement from physician or health care provider verifying disease history.
All students entering grades 6-12 must show proof of receiving one dose of Tdap vaccine (Tetanus, diphtheria, acellular pertussis) regardless of interval since last Dtap, DT or Td dose.

Vision Examination

New Student (First Entry Into an Illinois School)

Physical examinations must be completed on the ISBE child-wellness forms for all students entering K, 6, and 9. The physical exam must be dated less than 1 year prior to the first day of the current school year.
Documentation of immunizations must be provided.
Vision exam within 1 year prior to the first day of school is required.
Dental exam within 1 year prior to the first day of school is required.

Pre-Kindergarten & Speech Only

Children Entering Pre-K: Proof of immunization that complies with the ACIP recommendation for Hib vaccination. Children 24-59 months of age without series shall show proof of one dose of Hib vaccine at 15 months or older. Any child five years of age (60 months of age) or older shall not be required to provide proof of immunization with Hib vaccine.
Children Entering Pre-K: Proof of immunization that complies with ACIP recommendations for PCV. Children 24 to 59 months of age without a primary series of PCV, shall show proof of receiving one dose of PCV after 24 months of age. Any child five years of age (60 months of age) or older shall not be required to provide proof of immunization with PCV vaccine.
Physical examinations must be completed on the ISBE child-wellness forms. The physical exam must be dated less than 1 year prior to the first day of the current school year.

Transfer Students

Students coming from an Illinois School have 30 days from the date of enrollment to provide copies of mandated health requirements. (see grade level requirements below)
Students coming from OUT of state/OUT of country have 30 days from the date of enrollment to provide proof of a physical examination completed within the last 12 months and contains all State of Illinois examination requirements, all State of Illinois vaccine requirements and proof of an eye examination which was completed within the last 12 months.

Completed health documents can be emailed directly to districtnurseoffice@barrington220.org. You can also deliver the forms to your child's school or the District Administrative Center at 515 W Main St., Barrington, IL. There is a drop box by the front door. Please mark it to the attention of the District Nurse's Office.

Objection of Parent/Legal Guardian

Parents or guardians who object to examination(s) or immunization(s) on religious grounds are not required to submit their child or wards to either, provided they present a completed Illinois Certificate of Religious Exemption form. This form must be completed in its entirety by the parent/guardian and signed by the physician, advanced practice nurse of physician assistant performing the examination. The statement must set forth the specific religious belief which conflicts with the examination(s) and / or immunization(s). Each vaccine that is objected, must be listed. The form is available on the Public Health website at www.dph.illinois.gov (then search "religious").
